How clinical safety is measured.

Clinical safety in HealthOS is an institutional methodology, not a static audit. This page documents how Veronara assesses safety pre-deployment, monitors safety post-deployment, classifies incidents, and reports the institutional state to clinical leadership and the Clinical Advisory Board.

Pre-deployment assessment

Before a model or workflow goes live.

Every clinical-decision-support pattern, predictive model, or workflow change in HealthOS that touches patient care undergoes pre-deployment safety assessment. The assessment is institutional — performed by clinical reviewers with appropriate specialty representation — not algorithmic.

  • Scope of impact. What clinical decisions are affected, in which patient population, by what magnitude.

  • Failure mode analysis. What happens if the model fails, drifts, or is wrong. What is the worst-case clinical consequence.

  • Override pathway. How a clinician overrides the advisory. The override pathway is mandatory under the Advisory Principle.

  • Evaluation criteria. What signals constitute success, drift, and material failure — published before deployment.


Post-deployment monitoring

Continuous, not periodic.

Once deployed, clinical-decision-support patterns and models are monitored continuously. Drift signals are surfaced to clinical leadership; material drift triggers institutional review under the published evaluation criteria.

  • Override rate per recommendation.

  • Outcome correlation against the published evaluation criteria.

  • Demographic stability — performance variance across patient populations.

  • Clinician feedback channel — structured input from the Clinical Layer.


Incident classification

Severity tiers, named and dated.

Incidents affecting clinical safety are classified into severity tiers. Material incidents are publicly disclosed within 72 hours at /trust/incidents — with root cause, remediation, and data impact — and preserved without silent edit. The disclosure pathway is part of Veronara's institutional commitment.


Institutional review cadence

Quarterly, by the Clinical Advisory Board.

The Clinical Advisory Board reviews clinical safety state on a quarterly cadence. Material drift, incident clusters, and post-deployment performance trends are standing agenda items. Recommendations are recorded; recommendations not adopted are preserved with reason.


The Advisory Principle

Models advise. Clinicians decide.

The Advisory Principle is Veronara's institutional commitment that AI in HealthOS issues advisories, not decisions. The clinician is the decision-maker; the model is the advisor. Every advisory is logged, attributable to a specific model version, and subject to clinical review. Safety methodology depends on this architectural commitment.
